REFERRAL TO H-HHA SERVICES
For referral to these services, Veterans must first be seen by their primary care provider (PCP). This initial visit will initiate the following process:
- The PCP will make a referral to our service.
- H-HHA Social Work Coordinator will contact you to complete a phone assessment.
- Once approved, you will be referred to a community provider to start services.
